Important Information Regarding Health Coverage Tax Documents (Form 1095-C)
Dear Employee,
We are writing to inform you of an important update regarding the distribution of Form 1095-C for the 2025 tax year.
Due to recent federal legislation aimed at reducing paperwork and administrative burden, employers are no longer required to automatically distribute Form 1095-C to employees each year. Instead, the form will now be provided upon request only.
What Is Form 1095-C?
Form 1095-C contains information about the health insurance coverage that was offered to you during the year. This form is provided for your records and does not need to be submitted with your personal tax return.
How to Request Your Form 1095-C
If you would like to receive Form 1095-C for the 2025 tax year, you may submit a request on or after January 31, 2026, using one of the following methods:
